Yes. It is strongly recommended that you play the previous 3 games before playing MGS4, so you can get a better understanding of the events and storyline.
You could also read up on the events. There is a Metal Gear Solid Database available to download at the PlayStation store, free of charge. This database has in-depth articles on virtually everything in the Metal Gear universe and also includes a timeline of events and a character relationship diagram. The database also has a MGS4 lock, which prevents spoilers from MGS4 from being displayed. In order to unlock it, you must have a completed MGS4 game save.
Other places to read up on the Metal Gear series include Wikipedia and the Metal Gear Solid wiki.

Snake was designed this way. He wasn't never meant to live to his 43rd birthday. This is because his only purpose was to be a soldier, which is also why he can never reproduce. The Patriots didn't want him getting in the hands of their enemies, and should he be captured, he would quickly die of old age anyway.
One speculative biological explanation is that being a clone of Big Boss, Snake was born with cells that had shortened telomeres. If Big Boss was cloned at the age of 40, Solid Snake would be born with the telomeres of a 40 year old. By the time he turns 40, he has the telomeres of an 80 year old. In real life, Dolly the cloned sheep only lived half of her species' life expectancy, though it's unknown whether this was due to accelerated aging or unrelated health problems.

Yes. You can use CQC using a pistol/knife combination and a rifle/knife combination, something that couldn't be done in MGS3.
Yes. The Metal Gear Mk.II can be controlled remotely with the SIXAXIS. The SIXAXIS motion control can also be used to crouch and peak around corners while in first person mode. You can also shake the controller to return Snake's Octocamo to its default state.
An MGO starter pack will be included with MGS4. The starter pack includes 5 maps (Ambush Alley, Groznyj Grad, Midtown Maelstrom, Urban Ultimatum and Blood Bath). As time goes on, you'll be able to purchase more maps that will be available for download at the PlayStation store.

Both Konami and Sony have stated that this game is a PS3 exclusive. It will not be released on any other game console anytime soon. There is also an in-game joke by Otacon who tells Snake to change the game disc when the player is about half way through the game but then realizes that MGS4 benefits from the PS3's BluRay disc storage capabilities.
No. Two more games have been confirmed to be released in the next few years. There was Metal Gear Solid: Peace Walker for the PSP, and Metal Gear Rising: Revengeance for the PS3 and Xbox 360. Neither of these games have Solid Snake as the lead character though. Gamers play as Big Boss in Peace Walker, and Raiden will be the playable character in Rising. Kojima has stated that MGS4 will be the last game to feature Solid Snake as a playable character.
A Trophy Patch is now available
